Table of contents rebuilding multiple times.

I have a document with a table of contents but each time I try to update & replace instead of contents covering two pages which it should do it repeats and covers 25 pages and many of the page numbers are all mixed up

Hope someone can explain what is happening.

Sorry Sven - rather late I missed this reply.

After many attempts I managed to finish the document - not sure how but I got it off to the publishers for the deadline.
