Phrases don't appear in Index

I've added single word entries to an Index and it generates the index OK. However, if I highlight a phrase and add that as an entry, the phrases don't appear when the Index is generated - though the phrases appear in the Edit index entries list. Can anyone help me with what I'm doing wrong? Thanks. ...
